Employee Tuition Remission Benefit Request Form

This form must be completed each semester by SMC employees wishing to receive tuition remission benefits for SMC classes taken by themselves, their spouses or dependents.

Please enter any comments or questions you want to make the Human Resources or Financial Aid staff aware of as they process your request.

Upon submission by you, this form will automatically be routed first to Human Resources ( for review. The HR staff will process your request and then route your request onto the Financial Aid staff for final processing. You can expect an email response within ten (10) business days whether your request has been approved or denied.
